I hereby give my consent to have my child(ren) participate in the following activity of Calvary Chapel East Anaheim:
Talent Camp 2025.

I understand my child(ren) will observe and obey all posted rules and warnings, and further agree to follow any oral instructions and/or directions given by CCEA leaders, and/or the employees, representatives or agents of Calvary Chapel East Anaheim.

I recognize that there are risks involved in participating in Talent Camp and hereby assume all risk of injury, harm, damage, or death to my child(ren) in connection with his/her participation in Talent Camp. To the fullest extent permitted by law, I release Calvary Chapel East Anaheim, its trustees, officers, elders, directors, employees, agents and representatives from an injury, harm, damage or death which may occur to my child(ren) while participating in the activity and agree to save and hold harmless Calvary Chapel East Anaheim, its trustees, officers, elders, directors, employees, agents and representatives from any claims arising out of my child(ren)'s participation in Talent Camp.

Further, being the parent or legal guardian of the child(ren), I do consent to any medical, surgical, X-ray, anesthetic, or dental treatment that may be deemed necessary for my child(ren). I understand that efforts will be made to contact me prior to treatment but, in the event I cannot be reached in an emergency, I give permission to the activity leader to make the decisions necessary for treatment. Should there be no activity leader available, I give my permission to the attending physician to treat my child(ren). As parent or legal guardian, I understand that I am responsible for the healthcare decisions of my child(ren) and agree that my insurance plan is the primary plan to pay for the medical, dental, or hospital care or treatment that is given to my child(ren). Any insurance policy of the church or organization sponsoring this event will be used as the secondary coverage. I hereby release Calvary Chapel East Anaheim, its employees, agents, and volunteers, from any and all liability, claims, demands, cause of action and possible causes of action.

As the parent or guardian of a child/children at Calvary Chapel East Anaheim's Talent Camp, hereafter known as Talent Camp, agree to the following:

I understand that my child(ren) whose name(s) are registered for Talent Camp 2024 Soccer class will be visiting offsite to Yorba Regional Park July 21st-July 25th. I grant permission for my child(ren) to be shuttled to and from Calvary Chapel East Anaheim to participate and attend this event.

I understand that my child(ren) whose name(s) are registered for Talent Camp might be on camera, filmed, or photographed the week of Talent Camp. I understand that these videos and/or photographs may be used in the promotion of Talent Camp and Adventure Kids Ministry, either in print or on the Internet.

With my signature below I grant permission for my child(ren) to be photographed or filmed for print or electronic use in promoting Talent Camp and/or the Adventure Kids Ministry.
